It means there was some sort of problem with a page you were trying to get.

For example, if the server is running a perl program (did the address for the page you wanted end in .pl?), it might be that permissions might not be set correctly or something.

Internal Server Error is sort of a catch-all phrase for "Hey, user! Something messed up and it wasn't your fault."

I missed Duster's cool item in Chapter 5 because I didn't immediately go to Oshoe Castle and once I realized I needed to go there the game wouldn't let me
